A. Origineel op perkament, hoogte 183 mm., plica 17 mm., breedte 230 mm. Oud nummer CVII. Aan drie dubbele stroken perkament hingen de zegels van het gasthuis, van Arnold van Ghershoven, kanunnik van Tongeren en Balduinus, kapelaan van Offelken. Die drie zegels zijn verdwenen.
